Appointments & Office Hours

Scheduling Appointments

To schedule an appointment, please call our main office in Derry at (603) 421-9616 during regular office hours Monday through Friday from 9:00 am – 5:00 pm. We see patients on Monday, Tuesday, and Friday mornings in our Derry office, and all day Wednesday and Friday afternoons in our Salem office. Please call in advance for non-urgent appointments so that we may schedule a date and time to fit your convenience. We recommend that you schedule your next follow-up appointment at the conclusion of each office visit.

Please be aware that copies of relevant medical records including progress notes, lab test results, imaging studies, etc must be provided to us by your primary care physician or other referring physician in advance of your initial visit. In the event that copies of your medical records are not received at our office prior to your scheduled visit, we reserve the right to reschedule your appointment for a future date.

Please bring your health insurance card to your initial visit, as well as subsequent visits. In addition, please bring:

  • A list of prescription and over-the-counter medications you are taking
  • A list of known drug allergies
  • Relevant medical records and reports
  • Recent imaging tests such as x-rays and MRIs
  • Referral from your primary care physician if necessary

Please note that all patient telephone calls should be directed to our main office in Derry at (603) 421-9616 for patients seen at either our Derry or Salem office locations. All medical charts are stored in our Derry office.

Cancellations

We call to confirm your appointment one business day prior to your planned visit. In the event that you must reschedule an appointment, please notify our office at least 24 hours in advance.
